Catchment area for the Allendale Road Surgery

We welcome new patients living within our catchment area who wish to register with us. If you would like to register, please check you live within our practice area.

When you come to register, please bring the following to the practice:

  • Your NHS medical card (if you do not have an NHS medical card you will need to complete form (GMS1)
  • Proof of your address (eg a utility bill or bank statement no older than three months)
  • Your child’s Red Book if you are registering a child aged six years or under (we will take a copy of immunisation history)

If you are a family registering together, we need only to see proof of address for one adult family member although you will need to complete form GMS1 for all family members who do not have NHS cards. If you have recently has a baby, please register the baby’s birth and bring with you form FP58 which will be given at the time of birth registration.

When you attend the practice with your documents a receptionist will check your details and register you within the next 10 days. We like all our new patients to attend for a new patient health check with the nurse and this will be arranged at a time convenient for you.
